Most adults have back pain at some point in their lives. Most episodes resolve on their own within a few days. However, there are situations where delaying a doctor visit can be dangerous.

Normal pain vs warning signs

Muscle soreness after physical exertion, an awkward sleeping position, or exposure to cold is a normal body response. It typically eases with rest, responds to over-the-counter painkillers, and clears up within 3-7 days. Warning signs include pain that escalates, does not respond to treatment, or is accompanied by additional symptoms.

Red flags in back pain

Seek medical attention immediately if your back pain is accompanied by:

  • Fever above 38 degrees Celsius
  • Unexplained weight loss
  • Pain that worsens at night regardless of position
  • Recent trauma (fall, car accident)
  • Pain that persists after 4-6 weeks of conservative care

If you experience sudden loss of bladder or bowel control, progressive weakness in both legs, or numbness in the groin area, these are signs of cauda equina syndrome. Call emergency services immediately.

Neurological symptoms

Pain radiating below the knee often indicates nerve root compression. Watch for the following signs:

  • Shooting pain down the leg (sciatica)
  • Numbness or tingling in the foot or calf
  • Weakness when lifting the foot or walking on heels
  • Diminished reflexes in the leg

If your back pain has resolved and only mild muscle stiffness remains without neurological symptoms, observation and conservative care are appropriate: gentle activity, non-steroidal anti-inflammatory drugs, and gradual return to normal load.

For back pain, the initial evaluation is typically done by a neurologist or general practitioner. If a herniated disc, spinal stenosis, or spinal instability is found, the patient is referred to a neurosurgeon. Modern spinal neurosurgery offers minimally invasive procedures with short recovery times.

During the initial consultation, the doctor will take your medical history, perform a neurological examination, and order an MRI if needed. Based on the results, a treatment plan will be recommended, from physical therapy to endoscopic surgery. Do not delay your visit if symptoms are progressing.
