Sciatica: Causes, Symptoms, and Effective Treatment

Sciatica is pain caused by irritation or compression of the sciatic nerve, the largest nerve in the human body. The pain radiates from the lower back through the buttock and down the back of the leg, sometimes reaching the foot.

What is sciatica

The sciatic nerve originates in the lumbar spine and runs through the buttock area down each leg. When nerve roots in the lumbar region are compressed, a characteristic shooting pain travels along the nerve path. Sciatica is not a disease in itself but rather a symptom of an underlying spinal condition.

Causes of sciatic nerve pain

  • Herniated disc: the most common cause of sciatica
  • Spinal stenosis: narrowing of the spinal canal around the spinal cord
  • Spondylolisthesis: forward slippage of one vertebra over another
  • Piriformis syndrome: spasm of the muscle through which the nerve passes
  • Bone spurs: osteophytes that compress nerve roots

Symptoms

  • Shooting or burning pain from the lower back down the leg
  • Numbness or tingling in the leg or foot
  • Muscle weakness in the affected leg
  • Pain that worsens with prolonged sitting or standing
  • Pain typically affects only one side of the body

Seek emergency medical care if pain is accompanied by loss of bladder or bowel control, sudden severe weakness in the leg, or numbness in the groin area. These symptoms require urgent attention.

A neurosurgical consultation is recommended if pain persists beyond 4 weeks, worsens despite treatment, or interferes with daily activities. Early diagnosis helps prevent chronic nerve damage and improves outcomes.

Most cases of sciatica settle with conservative treatment, usually within 6 to 12 weeks. With timely care the outlook is generally good.

Treatment options

Conservative treatment

First-line therapy includes anti-inflammatory medications, muscle relaxants, and physical therapy. Therapeutic exercises focus on stretching and strengthening the back muscles. Epidural steroid injections are effective for significant pain.

Surgical treatment

When conservative therapy fails or neurological deficits progress, endoscopic microdiscectomy offers a minimally invasive option: removing the source of nerve compression through an incision of less than 1 cm with same-day discharge.
